Enrollment form is good for government and private

Fill up the form with complete information

After filling up, email back to us at info@hmocard.ph.

We will process and will send a you Billing Invoice with instuctions how to pay through official bak accounts of iCare. Bank options are BDO, BPI or Union Bank.

Send proof of payment to info@hmocard.ph.

After confirmation of payment, we will send the Health Vouchers to individual enrollees, validated and registered and ready to use on the effectivity dates indicated.

Timeline for transaction from the time we receive the enrollment list is 24-48 hours.
